Louisiana Student Residency Questionnaire Form

Students may qualify as homeless after Hurricanes Laura & Delta if their living situation has changed.

If you feel that your child may qualify under the criteria listed below, you have the
option of completing the form. It is not required.
- Are you living in your permanent residence (are you able to live in your home)?
  - If not, are you living in
    - Travel trailer/RV
    - Tent
    - Motel/Hotel
    - Doubled up – meaning you are living with other people such as friends or family
    - With an adult that is not a parent or legal guardian
    - Any other temporary housing
Students who fit the homeless guidelines may be eligible for assistance with school
related needs.
